122) Tubal Reversal Surgery More Successful Than In Vitro Fertilization
A member of the Chapel Hill Tubal Reversal Center Message Board inquires whether undergoing tubal reversal surgery would put her at risk for multiple birth if she becomes pregnant. Unlike in vitro fertilization (IVF) - the other treatment option for women who have undergone previous sterilization - tubal reversal surgery does not increase the risk of having twins or triplets unless there is a family history of multiples. In the most comprehensive study done to date of tubal reversal surgery and its resulting outcomes, data collected by Dr. Gary Berger and Dr. Charles Monteith showed that for the majority of women who have undergone a tubal ligation procedure and decide later they would like to have more children, tubal reversal surgery is a better option than IVF. Once the tubes are repaired with a reverse tubal ligation, there is a chance that pregnancy can occur naturally each month. With IVF, medications must be administered by injection and a minor surgical procedure performed each time pregnancy is attempted. Besides having higher success rates, the one-hour outpatient reverse tubal ligation procedure is less than half the cost of an IVF cycle.

125) FSH Recommended for Women in their Forties Planning Tubal Surgery
Prior to tubal surgery to reverse tubal ligation, Dr. Gary Berger and Dr. Charles Monteith recommend that women age 40 or older have a cycle day 3 FSH blood test. Follicle stimulating hormone (FSH) is a pituitary hormone that stimulates ovulation and can assess if a woman has healthy eggs in her ovaries. The test is usually performed on cycle day 3, but it can also be performed on day 2, 3, or 4 of a woman's menstrual cycle.
126) How Dr. Berger and Dr. Monteith Verify Tubal Patency after Reverse Tubal Ligation
Tubal ligation reversal involves microsurgical techniques to open and reconnect the fallopian tube segments that remain after a tubal ligation procedure. After opening the blocked ends of the remaining tubal segments, a narrow flexible stent is gently threaded through their inner cavities or lumens into the uterine cavity. This ensures that the fallopian tube is open from the uterine cavity to its fimbrial end. The newly created tubal openings are then drawn next to each other by placing a retention suture in the connective tissue that lies beneath the fallopian tubes. The retention suture avoids the likelihood of the tubal segments subsequently pulling apart. This is how the tubal reversal doctors with the most experience, Dr. Berger and Dr. Monteith at Chapel Hill Tubal Reversal Center, ensure the tubes are open after reversing tubal ligations.
