162) Methotrexate Helps Prevent Ectopic Pregnancy Complications After Tubal Reversal Surgery
Patients who have tubal surgery are at increased risk of tubal (ectopic) pregnancy. The complications of ectopic pregnancy can be minimized by early pregnancy monitoring. It is important to notify both us and your doctor as soon as your pregnancy test becomes positive and to follow our early pregnancy monitoring protocol. Atubal pregnancy will never lead to a live birth and must be treated. By following our early pregnancy protocol, patients are able to have abnormal pregnancies identified very early and this will decrease the possible complications of an ectopic pregnancy. Treatment of early ectopic pregnancies can be successfully accomplished with methotrexate medication. Methotrexate allows women to avoid surgery for ectopic pregnancy; however, side-effects are common and include nausea, painful cramping, and bleeding. The medication is quickly eliminated from the body and after successful treatment of an ectopic pregnancy, patients can try to conceive again without risks to future pregnancies. If a tubal pregnancy is allowed to progress without early diagnosis, it could eventually damage the fallopian tube and require surgical treatment, possibly involving removing the tube entirely.

This procedure is called tubouterine implantation. At Chapel Hill Tubal Reversal Center, Dr. Berger and Dr. Monteith have performed the tubouterine implantation procedure numerous times at for other situations and the success rate in those cases has been as much as 50% depending on the amount of healthy tube remaining after untying the tubes and the age of the patient.

The occurrence of tubal blockage in patients of Chapel Hill Tubal Reversal Center after a tubal reversal surgery is approximately 5%. If tubal blockage occurs after surgery and the tubal lengths are sufficient then Dr.Berger and Dr. Monteith may recommend repeat reversal surgery. This is not common. Often when tubal blockage is suspected, we commonly find errors in the reading of the hysterosalpingogram (HSG) test. We encourage our patients to send their films or CDs to our office for examination of the HSG. There is no charge to the patients for reading these x-rays. Our doctors know where the tubal anastomosis was done and how the test should appear following surgery.

172) Tubal Ligation Reversal Patient Asks About Discomfort After Surgery
A new message board member asks about the incision and discomfort following tubal reversal surgery. Her question is "How does recovery from tubal reversal compare to a c-section?" The microsurgery techniques used by Dr. Berger and Dr. Monteith during tubal reversal, begin with a small incision just above and parallel to the pubic bone. To minimize pain after the surgery, they avoid the use of metal retractors which are used by many surgeons. To further minimize pain, they apply local anesthesia to the skin, muscles, and connective tissues. These steps minimize tissue injury, bleeding, post-operative pain, and result in rapid patient recovery after the tubal ligation reversal. Most patients return to their hotel within two hours after their tubal reversal. Our patients regularly state how pleasantly surprised they are following tubal reversal surgery and how recovery is much easier than that of a c-section.

174) Early Pregnancy Monitoring After Tubal Reversal Surgery
A message board member of Chapel Hill Tubal Reversal Center shares her excitement over seeing her early pregnancy during her ultrasound visit. Dr. Berger and Dr. Monteith are able to repair the fallopian tubes in 98% of cases. The staff enjoys hearing these success stories from pregnancy reports which are shared on the message board. Part of our follow-up care after tubal reversal is recommending patients followtheir early pregnancies along with their local physician. We recommend monitoring early pregnancy with quantitative serum HCG (pregnancy hormone) test twice a week and scheduling a vaginal ultrasound exam when HCG level reaches 1500. When the HCG reaches 1500, a vaginal ultrasound exam should show a pregnancy within the uterus. If the ultrasound does not show a pregnancy within the uterus, patients may have to have a repeat HCG test and another vaginal ultrasound exam within 2 or 3 days. Sometimes the gestation sac may not be seen until HCG is about 2500 depending upon the quality of the ultrasound and the experience of the person performing the ultrasound examination. By following this early monitoring protocol our patients are able to detect a normal pregnancy at the earliest moment.

176) Essure Reversal Surgery Is Possible
Essure is a birth control device available as an alternative to traditional tubal ligation methods. The Essure is a spring-like device inserted through the cervix and into the tubal openings using a small camera. The device expands to fill the tubal opening and causes scar tissue to form. The scarring from the Essure results in closure of the narrowest portion of the tubes as they travel through the uterine wall. Essure sterilization can be reversed, but the Essure insert cannot be simply pulled out because the scar formation seals the insert inside the tube. Dr. Gary Berger and Dr. Monteith are performing tubal reimplantation procedures to reverse Essure sterilization. Tubal reimplantation provides a 30-50% chance of pregnancy success. Tubal reversal surgeries are performed 5 days a week. Chapel Hill Tubal Reversal Center is the only medical facility exclusively dedicated to tubal ligation reversal.
